What is chronic sinusitis?
Chronic sinusitis, also called chronic rhinosinusitis or CRS, means the sinuses and nasal passages stay inflamed for 12 weeks or longer, even with treatment. A regular sinus infection usually clears up in about 4 weeks. This one doesn't. Common symptoms include a blocked or stuffy nose, drainage down the back of the throat, pressure or pain in the face, and a weaker sense of smell. Nearly 29 million US adults, about 1 in 9, reported a sinusitis diagnosis in the past year.
There's no staging system for this condition. Instead, doctors split it into two types, and trials recruit for each separately. With nasal polyps (CRSwNP) means soft growths form inside the nose and sinuses, driven by a type of inflammation called type 2, or eosinophilic. That same type of inflammation drives asthma and atopic dermatitis too, which is why the three conditions often show up together. Without nasal polyps (CRSsNP) means the same long lasting inflammation, but no growths. Knowing which type you have helps you figure out which studies fit you, since most trials list the type right in their eligibility rules. Trials in this field usually track two things: the size of the polyps, seen with a small camera, and how blocked your nose feels day to day.
Can chronic sinusitis be cured?
There is no known permanent cure today. Treatment aims to control the inflammation and keep symptoms down over the long term, and for many people that means managing it for years rather than clearing it once. Research is moving quickly though. Five medicines are now approved in the US specifically for this condition, and every one of them came out of a clinical trial.
What are the symptoms of chronic sinusitis?
The four main symptoms are a blocked nose, mucus draining from the nose or down the throat, pressure or pain in the face, and a weaker sense of smell. The key part is time. They have to last 12 weeks or more, even with treatment, before doctors call it chronic. A headache on its own is not enough. Doctors tie headaches to this condition only when they come together with the blockage, the drainage, and the loss of smell.
Types of clinical trials for chronic sinusitis
Researchers use several types of clinical trials to study chronic sinusitis, each looking at a different piece of the condition, from diagnosis to new treatments to surgical alternatives.
Diagnostic trials
These trials work on improving how doctors identify and classify chronic sinusitis, especially telling CRSwNP and CRSsNP apart and spotting the type 2, or eosinophilic, inflammation that drives many cases. Researchers test tools like nasal endoscopy, imaging, and blood or tissue biomarkers to see which ones predict a patient's disease type and likely response to treatment.
The goal is earlier, more accurate diagnosis, so patients get matched to the right treatment sooner instead of cycling through options that were never a good fit for their type of inflammation.
Interventional trials
Interventional trials test new medications, most often biologics that target the specific inflammation pathways behind CRSwNP, alongside nasal sprays, steroids, and other drug therapies. Participants receive the treatment or a placebo under close monitoring, and researchers track changes in polyp size, nasal blockage, and sense of smell.
These trials are also where new or improved sinus surgery techniques get tested, comparing surgical outcomes to medication alone or to older surgical approaches.
Device and procedural trials
These trials evaluate physical tools used to manage chronic sinusitis, such as balloon sinuplasty devices, drug-eluting sinus stents, and other in-office procedures. Researchers assess how well these devices open blocked sinuses, deliver medication directly to the area, and hold up over time compared to surgery or medication alone.
Observational studies
Observational studies follow people with chronic sinusitis over time without testing a new treatment, watching how the disease naturally progresses, how CRSwNP and CRSsNP differ in their course, and what factors predict flare ups or the need for surgery. This kind of long term data helps researchers understand the disease well enough to design better interventional trials down the line.
Who can participate in chronic sinusitis clinical trials?
Most studies sort people by one simple question: do you have nasal polyps or not? Trials for the polyps form (CRSwNP) usually look for adults whose polyps came back or stayed despite treatment, and many ask that you have already tried steroid sprays or had sinus surgery. Trials for the form without polyps (CRSsNP) are newer ground, since it got its first approved drug only in 2024, and researchers need participants there. Some studies recruit people who are considering sinus surgery or already had it. Others are observational and just track how your symptoms change over time, with no new treatment involved. A few recruit people who have chronic sinusitis alongside another condition, such as cystic fibrosis.
