What is migraine?
Migraine is a brain condition, not a blood vessel problem or a stress reaction. An attack usually brings throbbing pain on one side of the head, along with nausea and sensitivity to light and sound, and it can last from a few hours to three days. About 1 in 4 people with migraine get an aura first, visual disturbances like flashing lights or zigzag lines that come before the pain. Doctors sort migraine by frequency: episodic means fewer than 15 headache days a month, chronic means 15 or more, with at least 8 of them migraine days, for more than three months.
For most of medical history, migraine was treated with medicines borrowed from other conditions: blood pressure pills, antidepressants, epilepsy drugs. That changed in 2018, when the first medicine designed specifically for migraine was approved. It blocks a molecule called CGRP, which the brain releases during an attack. There are now four CGRP antibodies given by injection and several CGRP blocking pills, called gepants, and they treat both attacks and prevention. In a 2025 real world study of more than 2,000 patients, about 47 percent of people on a CGRP antibody had a meaningful response at three months, compared with about 35 percent on traditional preventives.
Types of migraine clinical trials
Prevention trials. The largest group. These test whether a medicine reduces the number of migraine days per month, usually over 12 weeks or longer. Some compare two approved preventives head to head to learn which works best for whom.
Acute treatment trials. These test medicines taken during an attack and measure whether pain is gone at two hours and whether it stays gone.
Real world and safety studies. After a drug is approved, these follow people using it in everyday life, for example women using a specific medicine around their period, or people with heart risk factors.
Studies of specific groups. Menstrual migraine, chronic migraine that has resisted other treatments, and childhood migraine each have dedicated studies, because they behave differently and were long understudied.
Non drug and mechanism studies. Some trials test things like cocoa extract or probiotics, others use brain scans to understand why migraine becomes chronic. No treatment is promised in mechanism studies, but they shape what gets tested next.
Who can participate in migraine clinical trials?
Migraine studies almost always start with your attack frequency: how many migraine days and headache days you have per month, usually confirmed with a headache diary during a screening period. Second, whether your migraine is episodic or chronic, since most trials enroll one or the other. Third, your treatment history: many prevention trials want people who have tried and stopped two or more preventives, others want people who have never tried one. Some studies focus on specific groups like menstrual migraine or people with medication overuse. Eligibility always varies by study.
Episodic and chronic migraine, and why the line matters for trials
Migraine has no stages, but the episodic versus chronic split matters enormously. Chronic migraine means 15 or more headache days a month, at least 8 of them with migraine features, for over three months. It affects roughly 1 to 2 percent of people, tends to come with more depression, anxiety, and disability, and often develops when episodic migraine slowly escalates, a process called chronification that researchers are actively studying. For trials, the label decides your eligibility: nearly every study says episodic or chronic in its title. Knowing your monthly count tells you at a glance which studies fit.
