What is COPD?
COPD stands for chronic obstructive pulmonary disease. It's actually an umbrella term for two conditions: emphysema, where the tiny air sacs in the lungs get destroyed, and chronic bronchitis, where the airways stay inflamed and clogged with mucus. Both make it harder to breathe. Smoking is the main cause in the US, but not the only one. Air pollution, workplace dust and fumes, and a genetic condition called alpha-1 antitrypsin deficiency can cause it too. Some people with COPD never smoked.
A few things make COPD research unusual. Most studies don't test a drug at all, they test breathing devices, oxygen equipment, and rehab programs. Drug trials tend to run long, often about a year per participant, since what they're tracking is flare ups, and those don't happen on a schedule. Researchers are also working to close a gap: COPD rates run higher in rural areas and in American Indian and Alaska Native communities, groups that trials have historically missed.
What are the signs and symptoms of COPD?
Common symptoms include shortness of breath, especially during physical activity, an ongoing cough or a cough that brings up a lot of mucus, wheezing, chest tightness or heaviness, and tiredness. The ongoing cough is often the first symptom people notice. Some of these symptoms overlap with asthma, and the two conditions can even occur together, a mix doctors call asthma-COPD overlap. And at the start, COPD may cause no symptoms at all, or only mild ones, which is one reason stage 1 so often goes unnoticed.
Can COPD be reversed?
No. COPD cannot be reversed, and there is no cure yet. The narrowing in the airways is persistent: it does not go back to normal, even with medicine. But treatment and lifestyle changes such as quitting smoking can slow the disease down and ease symptoms.

Who can participate in COPD clinical trials?
It depends on the study. Most COPD drug studies ask for a few of the same things: a diagnosis confirmed by spirometry, your GOLD stage, your smoking history in pack years, and how many flare ups you had in the past year. A smoking history of 10 or more pack years is a common cutoff. Some of the newer biologic studies also need a blood test result, because those medicines only work for people with one specific inflammation pattern. Device, monitoring, and rehab studies on this page usually have simpler requirements, and later stage COPD does not shut the door, studies in this field often look for people with more severe disease. Researchers also want to enroll more people from rural areas and from American Indian and Alaska Native communities, which studies have long struggled to reach. Eligibility always varies by study.
