What is osteoarthritis?
Every joint has a layer of smooth cartilage that lets bones glide against each other. In osteoarthritis, or OA, that cartilage wears thin, the bone underneath changes, and the joint lining can get inflamed. The result is pain that gets worse with use, stiffness after resting, and over time less movement. OA is not simply aging: injury, extra body weight, genes, and repetitive joint stress all raise the risk, and more than half of people with symptomatic knee OA are younger than 65. It is also not the same as rheumatoid arthritis, which is an immune system disease.
The scale is huge. More than 500 million people worldwide have OA, about 62 percent of them women, and in the US it costs an estimated 137 billion dollars a year. Yet OA has something almost no other major disease has: zero approved medicines that change its course. Pain relievers, injections, physical therapy, and weight loss help symptoms, and joint replacement works well when nothing else does, but nothing on the market slows the cartilage loss itself. Several attempts over the past decade failed in late stage trials. The most promising recent result came from an unexpected direction: in a 2024 trial, the weight loss medicine semaglutide cut knee OA pain substantially more than placebo in people with obesity. It is not approved for OA, but it opened a new research front.
Can osteoarthritis be cured or reversed?
Not with any approved treatment. No medicine has ever been shown to slow or reverse cartilage loss in OA. Current care treats pain and function, and joint replacement is the option when that fails. Disease modifying drugs are the main goal of ongoing research.
Types of osteoarthritis clinical trials
Disease modifying drug trials. The holy grail of OA research: medicines meant to slow cartilage loss rather than mask pain. Several are in trials, none is approved, and every attempt so far has taught the field something about why this is hard.
Pain treatment trials. New pain medicines, injections into the joint, and non opioid approaches, including cannabinoid based treatments. These are measured by how much pain and function improve over weeks to months.
Weight loss and metabolic studies. A fast growing area since 2024. Trials test GLP-1 medicines in people who have both obesity and knee OA, asking whether treating the metabolic side reduces joint pain.
Procedure and device trials. Genicular artery embolization, which blocks small blood vessels feeding an inflamed knee, low dose radiation, cartilage repair procedures, and powered braces are all under study for people who want to avoid or delay joint replacement.
Exercise, rehab, and prevention studies. How much exercise helps, how to get people ready for or recovering from joint replacement, and whether OA can be prevented in people at high risk after a knee injury.
Who can participate in osteoarthritis clinical trials?
OA studies usually recruit around three things. The joint: most trials focus on the knee, fewer on the hip or hand, and they name it in the title. The severity, usually graded 1 to 4 on an X ray using the Kellgren Lawrence scale, with most drug trials enrolling grades 2 and 3, moderate disease with cartilage still left to protect. And your pain level, often confirmed with a questionnaire during screening. Some studies also require a certain body mass index, especially the weight loss medicine trials, or a recent knee injury for prevention studies. Eligibility always varies by study.
The grades of osteoarthritis, and why they matter for trials
Doctors grade OA on X ray with the Kellgren Lawrence scale:
- Grade 1. Doubtful: tiny bone spurs, joint space looks normal.
- Grade 2. Mild: clear bone spurs, joint space still mostly preserved.
- Grade 3. Moderate: several bone spurs and visible narrowing of the joint space.
- Grade 4. Severe: large bone spurs, joint space nearly gone, bone changes.
X ray grade and pain do not always match: some people with grade 4 knees walk comfortably, some with grade 2 knees are in constant pain. That is why trials usually require both a grade and a pain threshold. Most disease modifying drug trials enroll grades 2 and 3, because there needs to be cartilage left to save. Grade 4 does not close doors, though: procedure studies, replacement readiness programs, and device trials are built for exactly that group.
Do osteoarthritis trial participants get paid?
