What is prostate cancer?

The prostate is a walnut sized gland below the bladder that makes part of the fluid in semen. Prostate cancer starts when cells there grow out of control. It is usually slow growing, often has no symptoms for years, and is most often found through a PSA blood test rather than through anything a man notices. The median age at diagnosis is 68, and it is rare before 40. Black men are about 70 percent more likely to be diagnosed and twice as likely to die from it as white men, a gap researchers are actively studying.

The Importance of Inclusion & Diversity in Clinical Trials

What makes prostate cancer different from most cancers is the range. At one end, many low risk cancers can be safely watched with regular PSA tests and scans, an approach called active surveillance, instead of being treated at all. At the other end, cancer that has spread beyond the prostate is treated with hormone therapy, newer androgen receptor blockers, chemotherapy, and, since 2022, a targeted radioactive medicine called Pluvicto that homes in on a protein on prostate cancer cells. Its approval was expanded in March 2025 to earlier use before chemotherapy, and in July 2026 to metastatic hormone sensitive disease.
