What an Anemia Diagnosis Alone Doesn't Reveal

Anemia by type: how each one is treated

Anemia isn't really one disease, it's more like a symptom doctors give a name to. It means someone doesn't have enough healthy red blood cells, or enough hemoglobin in them, to carry oxygen around the body well. That one symptom can come from a dozen genuinely different underlying problems, and the right treatment depends completely on which one someone actually has. Here's how the major types are treated, and what's changed recently for each.

Iron and B12 deficiency

Most anemia comes down to low iron, usually from blood loss, pregnancy, or the body not absorbing enough of it. Iron pills are usually the first thing tried. If pills alone aren't enough, an iron IV can now deliver a full dose in just one or two visits instead of many. A smaller number of cases come from low vitamin B12 instead, which is treated differently, usually with B12 shots or high-dose B12 pills rather than iron.

Sickle cell disease

This one is genetic. It makes red blood cells stiff and shaped like a sickle instead of round, which causes sudden, painful attacks. Hydroxyurea is the main long-term treatment, often paired with a drug called Endari to cut down on how often those painful episodes happen. Another drug, Adakveo, is still approved and used in the US, though European regulators pulled it in 2023 after a larger follow-up study didn't confirm it worked as well as the original study suggested.

The bigger news is gene therapy. Two treatments, Casgevy and Lyfgenia, approved in late 2023, actually edit a person's own cells so they stop making sickled blood cells in the first place. It's a one-time, intensive process, but it's aiming at something much closer to a cure, not just managing symptoms.

Thalassemia

This is also genetic. It's usually managed with regular blood transfusions, plus medication to clear out the extra iron that all those transfusions build up over time. A drug called Reblozyl cuts down how often someone needs transfusions, and there's also a gene therapy option for people who need transfusions regularly. In February 2026, the company that makes Reblozyl announced promising early results treating a related form of thalassemia that's historically had fewer treatment options.

Aplastic anemia

This happens when the bone marrow stops making blood cells altogether. Treatment usually means calming down the immune system, so it stops attacking the marrow, combined with a drug called Promacta to help new blood cells grow. For people who qualify, a bone marrow transplant can actually cure it outright.

Anemia from kidney disease

Damaged kidneys stop producing enough of the hormone that tells the body to make red blood cells. This is usually treated with injections. A newer option, Vafseo, approved in 2024, is a once-daily pill instead, giving people on dialysis a real alternative to regular injections.

What researchers are studying now

There's also a promising new development for sickle cell disease. A drug called mitapivat, already used for a different type of anemia, was tested in a large study for sickle cell disease too. It helped a good number of people, about 4 in 10, get a meaningful boost in their hemoglobin levels, something almost no one on the placebo saw. It didn't clearly cut down how often people had painful flare-ups, though. Based on these results, the FDA is now reviewing it on a faster track, with a decision expected by November 2026. If it gets approved, it would be the first pill of its kind made specifically for sickle cell disease.

Can anemia be a sign of something more serious? Sometimes. Unexplained anemia, especially in adults without an obvious cause like heavy periods or pregnancy, is one of the reasons doctors sometimes look further into things like internal bleeding or an underlying long-term health condition.
