What is bladder cancer?

Bladder cancer usually starts in the urothelial cells lining the inside of the bladder, and the first sign is often blood in the urine. About 3 in 4 cases are non muscle invasive, confined to the inner lining; these are usually removed through a scope and often treated with BCG, a weakened bacterium placed in the bladder to trigger an immune response. They frequently come back, so follow up is lifelong. About 3 in 10 are muscle invasive, treated with chemotherapy and removal of the bladder or with bladder sparing chemoradiation. Five year relative survival is about 79 percent overall in US data, largely because about half of cases are found before they spread beyond the inner lining. Smoking is the biggest risk factor, followed by workplace chemicals; see our smoking cessation trials.
