What is C. diff?

C. diff is an infection of the large intestine caused by Clostridioides difficile, a spore forming bacterium that produces toxins that damage the bowel lining. It causes watery diarrhea, abdominal cramping, fever, and nausea, and in severe cases a dangerous swelling of the colon. The biggest risk factor is recent antibiotic use, which clears the normal gut bacteria that keep C. diff in check; being over 65, a recent hospital or nursing home stay, a weakened immune system, and acid suppressing medicines also raise risk. The spores spread easily on hands and surfaces, which is why C. diff is a leading hospital acquired infection.

Treatment is an antibiotic that targets C. diff itself, most often fidaxomicin or vancomycin, with fidaxomicin now preferred because fewer people relapse after it. Recurrence is the central problem, and for people whose infection keeps coming back, two microbiota based products approved in 2022 and 2023 restore healthy gut bacteria, one given rectally and the first taken as capsules. Bowel inflammation from other causes is covered on our ulcerative colitis and IBS pages.
