What is high blood pressure?
Blood pressure is the force of blood against artery walls, written as two numbers: systolic, when the heart beats, over diastolic, when it rests. Under US guidelines, high blood pressure, or hypertension, is a reading consistently at or above 130 over 80. Over years, that extra force stiffens arteries and strains the heart, and it is the leading risk factor for stroke and a major cause of heart failure, heart attack, and kidney disease. Most of the time it has no symptoms, which is why it is called a silent condition and why it is found by measurement rather than by how you feel. Headaches, dizziness, or nosebleeds are usually not caused by ordinary high blood pressure, though very severe readings can cause them.
Treatment starts with lifestyle changes, and for about 4 in 5 adults with hypertension guidelines also recommend medicine, usually from several well established classes. The problem is not a lack of drugs but staying on them: daily pills are easy to miss, and control rates have stagnated. Research is answering that in three ways. Long acting medicines such as zilebesiran, which silences the liver's production of a blood pressure hormone with an injection every six months, are in Phase 3 outcome trials measuring heart attacks and strokes. A new class of pills that blocks aldosterone production, including baxdrostat, is in Phase 3, and one drug for hard to treat hypertension, aprocitentan, was approved in 2024. Device procedures that quiet the nerves to the kidneys have been approved since 2023 and are being refined. Types of high blood pressure clinical trials
Long acting medicine trials. Injections given twice a year, tested against placebo on top of usual medicines, including large trials measuring heart attacks, strokes, and kidney outcomes rather than just blood pressure numbers.
New pill classes. Aldosterone synthase inhibitors and other new mechanisms, often in people whose pressure is uncontrolled on standard drugs.
Device and procedure trials. Renal denervation using ultrasound or radiofrequency through a catheter, and pacemaker based approaches, for people who cannot get control with medicines; see medical device clinical trials.
Exercise and lifestyle trials. Isometric handgrip devices, diet studies, and timing of medicine doses, often enrolling people with mildly elevated pressure.
Pregnancy related hypertension. Studies of blood pressure medicine after preeclampsia to protect the heart long term.
Implementation studies. Pharmacist led care, home monitoring, and programs that reach people who are not on treatment at all.

Who can participate in high blood pressure clinical trials?
Most medicine trials enroll adults whose blood pressure is above a set threshold despite taking one or more standard medicines, confirmed with repeated measurements and often 24 hour monitoring. Outcome trials add a requirement for high cardiovascular risk, such as prior heart disease or diabetes. Device trials usually require uncontrolled pressure on several medicines and a scan of the kidney arteries. Lifestyle and exercise studies often enroll people with mildly elevated pressure who are not yet on medicine. Most drug trials are placebo controlled on top of usual treatment, so no one stops their current medicine without a plan. Eligibility always varies by study.
What is the newest treatment for high blood pressure?
Aprocitentan, approved in 2024 for resistant hypertension, is the newest medicine. In trials, zilebesiran, an injection that lowers blood pressure for up to six months, is in Phase 3, and baxdrostat, a new aldosterone blocking pill, is in Phase 3 outcome studies.
Blood pressure categories, and why they matter for trials
Normal: below 120 over 80. Elevated: 120 to 129 over less than 80, where lifestyle trials often enroll. Stage 1 hypertension: 130 to 139 over 80 to 89. Stage 2 hypertension: 140 over 90 or higher, where most medicine trials enroll. Resistant hypertension: pressure above goal despite three medicines including a diuretic, the target of device and new drug trials. Study listings state the range they need, usually as a systolic number, and whether you must already be on treatment. Knowing your usual home readings and your current medicines tells you at a glance which studies fit.
Do high blood pressure trial participants get paid?
Some studies offer compensation for time and travel, and study related care, including home blood pressure monitors when the study provides them, is typically provided at no cost to participants.
