What is postpartum depression?

Postpartum depression is a major depressive episode that begins during pregnancy or in the year after childbirth, most often within the first weeks or months. It is different from the baby blues, the tearfulness and mood swings that affect most new mothers in the first two weeks and pass on their own. Postpartum depression lasts longer, usually at least two weeks, and is deeper: persistent sadness or emptiness, loss of interest, severe anxiety or panic, trouble bonding with the baby, guilt, changes in sleep and appetite beyond what a newborn causes, and sometimes thoughts of harming oneself or the baby. Those last thoughts are frightening and common, and they are a reason to seek help immediately, not a sign of being a bad parent. Fathers and partners can develop it too, with estimates around 1 in 10.

The causes are a mix of the steep hormonal drop after delivery, sleep deprivation, a history of depression or anxiety, lack of support, and stressful circumstances. Without treatment it can last months or longer and affects the child's development as well as the mother. Treatment works: psychotherapy, standard antidepressants, and support all help, and the two newer medicines act on a different brain system than SSRIs. Brexanolone, approved in 2019, is a 60 hour hospital infusion; zuranolone is a two week course of pills taken at home, approved in 2023 after trials in women with severe postpartum depression showed significant improvement by day 15 that lasted after the course ended. Both are neurosteroids, a new class of psychiatric medicine, and trials are now testing whether an oral version of brexanolone, ketamine, and rapid magnetic stimulation can help more people faster.
