What is naltrexone, and how does it work?
Naltrexone is a prescription medicine used to reduce drinking. It is an opioid antagonist, which means it blocks opioid receptors rather than activating them. Alcohol produces part of its pleasurable effect through those same receptors, so with naltrexone in the way, drinking simply delivers less of what the brain is chasing.
The practical result is usually a reduction in how much someone drinks rather than an immediate stop. That is a legitimate goal in itself, and it is what most research now measures.
Types of naltrexone clinical trials
Matching studies
Naltrexone works very well for some people and barely at all for others. A significant share of current research tries to predict who responds, using genetics, drinking pattern or family history.
Dosing and timing studies
Comparing the daily tablet with the monthly injection, testing different doses, and testing targeted approaches in which the tablet is taken before drinking rather than every day.
Combination studies
Naltrexone paired with another medicine, or with a behavioural programme, to see whether the combination outperforms either alone.
Access and delivery studies
Getting naltrexone to people through primary care, telehealth and pharmacies rather than specialist clinics, since access is clearly the bottleneck.
Studies in specific groups
People who also use other substances, people with liver conditions, and people with a co-occurring mental health condition, each of whom were often excluded from earlier research.
Who can participate in naltrexone clinical trials?
Most studies enrol adults with alcohol use disorder. Many accept people who are currently drinking, because reduced drinking is the outcome being measured, so you do not need to have stopped first.
Because naltrexone blocks opioid receptors, studies exclude people currently taking opioid medication, and require a period without opioids before starting. Liver function is usually checked at screening. Eligibility always varies by study.
Is naltrexone used for anything other than alcohol?

How to enroll in a naltrexone clinical trial
Identify your trial. Use the filters to combine alcohol use disorder with naltrexone. Titles usually say which form is being tested and what the study is comparing it against.
Check the setting. Some studies run in clinics, others largely by telehealth. The study details say how visits work and how the medicine is provided during the study.
