What is glioblastoma?
Glioblastoma, or GBM, is a grade 4 glioma, a cancer of the brain's supporting glial cells. It grows fast, sends microscopic tendrils into surrounding brain tissue so it can never be fully removed by surgery, and almost always returns after treatment. About 3 in 100,000 Americans are diagnosed each year, roughly 12,000 people, most of them between 45 and 70, and men slightly more often than women. Symptoms depend on where the tumor sits and usually appear over weeks: headaches that are new or worsening, seizures, personality or memory change, weakness on one side, or trouble with speech or vision. It is diagnosed with MRI and confirmed by surgery, which also removes as much tumor as is safe.
Two features shape prognosis and trials. Tumors with a mutation in a gene called IDH behave less aggressively and, since 2021, are classified separately; true glioblastoma is IDH wild type. And tumors whose MGMT gene is switched off respond better to temozolomide. Most glioblastomas arise with no known cause: they are not inherited except in rare genetic syndromes, and no lifestyle factor has been shown to cause or prevent them, apart from prior radiation to the head. Treatment after diagnosis is surgery, then six weeks of radiation with daily temozolomide, then monthly temozolomide, and for some people a wearable device that delivers electric fields to the scalp. When the tumor returns, options are limited, which is why recurrent glioblastoma is one of the most active areas of early phase research in all of oncology. Types of glioblastoma clinical trials
Newly diagnosed trials. New drugs added to radiation and temozolomide from the start, when the tumor is smallest and the immune system strongest. These are the trials most likely to change the standard of care.
Immunotherapy trials. Vaccines made from the tumor's own proteins, T cell engagers that link immune cells to a glioblastoma protein such as EGFRvIII, and CAR T cells, mostly in early phase.
Gene therapy trials. Viral vectors placed into the tumor at surgery that deliver genes designed to kill cancer cells or alert the immune system; see gene therapy clinical trials.
Device and radiation trials. Personalized tumor treating fields planning, radiation tiles placed in the surgical cavity, and targeted radioactive drugs; see medical device clinical trials.
Repurposed and combination drugs. Existing medicines, from diabetes drugs to older chemotherapies, tested with temozolomide for recurrent disease.
Imaging and decision studies. Trials testing whether advanced MRI can tell true tumor growth from treatment effects, one of the hardest calls in glioblastoma care.

Who can participate in glioblastoma clinical trials?
Trials divide sharply between newly diagnosed and recurrent disease. Newly diagnosed studies usually enroll within a few weeks of surgery, before radiation begins, and require a confirmed IDH wild type diagnosis, often with MGMT status. Recurrent studies enroll after the tumor has grown back on MRI, sometimes requiring a second surgery so the study treatment can be placed in the cavity or tumor tissue can be tested. Almost all require that you can manage daily activities, measured by a performance score, and stable or low steroid doses, because steroids suppress the immune responses many trials depend on. How does glioblastoma affect the body as it progresses?
The tumor grows within the brain and raises pressure inside the skull, which causes worsening headaches, drowsiness, weakness, seizures, and eventually loss of consciousness. It rarely spreads elsewhere. Steroids, anti seizure medicines, and palliative care teams manage these effects, and hospice care focuses on comfort in the final stage.
How glioblastoma is classified, and why it matters for trials
Glioblastoma is not staged like other cancers, because it almost never spreads outside the brain. It is always grade 4, the highest. What matters instead is the point in treatment and the tumor's molecular features.
Newly diagnosed. After surgery and before or during chemoradiation. The setting for trials adding new drugs to standard care.
Recurrent or progressive. Regrowth on MRI after initial treatment, typically within a year. The setting for most early phase and device trials.
Molecular features. IDH wild type defines glioblastoma; MGMT methylation predicts temozolomide benefit and is often required or stratified in trials; EGFRvIII and other markers select immunotherapy trials.
Study titles say "newly diagnosed" or "recurrent," and often "MGMT unmethylated" or a marker name. Your pathology report and most recent MRI report tell you which studies fit.
How to enroll in a glioblastoma clinical trial
Identify your trial. Use the filters and start with "newly diagnosed" or "recurrent." Because the window for newly diagnosed trials is short, look before radiation starts if you can.
Select your preferred location. Enter your city or state, then widen the radius; many glioblastoma trials run at a single academic center, and sponsors often help with travel.
