What is multiple sclerosis?
In MS, the immune system attacks myelin, the insulation around nerve fibers in the brain and spinal cord. Damaged spots, called lesions, slow or block nerve signals, which is why symptoms range from fatigue and numbness to vision problems and trouble walking. Symptoms and pace differ so much between people that MS is often called a disease with a thousand faces. It is not contagious, and it is not directly inherited. Worldwide about 2.9 million people live with MS, someone is diagnosed roughly every five minutes, and the average age at diagnosis is 32.
Research in this field carries one of the most striking findings in modern medicine: a 2022 study of more than 10 million US military members found that infection with the Epstein Barr virus, the common virus behind mononucleosis, raises MS risk 32 times, making it the leading suspected cause. Meanwhile treatment keeps expanding, with more than 20 approved disease modifying therapies, though the progressive forms still have almost none.
The types of MS, and why they matter for trials
- CIS (clinically isolated syndrome). A first episode of symptoms. Not everyone with CIS goes on to develop MS.
- Relapsing remitting MS (RRMS). Attacks of symptoms followed by recovery periods. About 85% of people start here, and most approved medicines were built for this form.
- Secondary progressive MS (SPMS). A phase after RRMS where disability gradually grows. When it advances without relapses, no approved therapy currently exists for it.
- Primary progressive MS (PPMS). Steady worsening from the start, in roughly 10 to 15% of people. One drug, ocrelizumab (Ocrevus), has been approved for it since 2017.
The practical point for trial seekers: almost every MS study names the type it enrolls, plus a disability score called EDSS. Knowing your type tells you at a glance which studies fit you, and the progressive forms are where researchers most need volunteers right now.

These studies might involve lifestyle changes, medications, or other interventions that could potentially prevent the onset or reduce the progression of MS.
Rehabilitation trials
Rehabilitation trials research ways to improve daily functioning and quality of life for those diagnosed with MS. These trials might test physical therapy programs, cognitive rehabilitation techniques, or novel assistive technologies designed to help manage the symptoms of MS.
Trials for emerging therapies
These trials explore the potential of new and emerging therapies in MS treatment, such as stem cell therapy, neuroprotective strategies, and novel immunotherapies. The goal is to find breakthrough treatments that can significantly alter the course of MS.
Who can participate in MS trials?
MS studies usually recruit around four things: your MS type (relapsing or progressive), your disability level measured by the EDSS score, your relapse history, and which disease modifying therapies you have already tried. Some studies enroll only newly diagnosed people, others focus on progressive MS, where the need is greatest. Observational studies often have the widest doors, including studies of people at the earliest stage of disease. Eligibility always varies by study.
