What is vaping, and why is it studied?

Vaping means inhaling aerosol from an e-cigarette, which heats a liquid usually containing nicotine, flavorings, and carriers like propylene glycol. Most devices sold today are disposables, and the nicotine in one can equal a pack of cigarettes or more. Nicotine is highly addictive, and teen brains are especially sensitive to it. In 2024, 5.9 percent of middle and high school students vaped, down from over 20 percent in 2019, while young adults 18 to 25 vape the most of any age group, about 14 percent. About 6.5 percent of US adults vaped in 2023, many of them current or former smokers.

Vaping is likely less harmful than smoking, which is why some adults use it to switch, and the FDA has authorized a small number of e-cigarette products on that basis. But less harmful is not harmless: the aerosol contains ultrafine particles, metals, and lung irritants, and since vaping is only about 15 years old, its long term effects on cancer and heart disease are still unknown. No medication is yet approved to help people quit vaping, though cytisinicline has a separate research program for it, and behavioral support like text based quit programs has the best evidence so far for teens. Clinical trials are working on both gaps right now.
