What are remote and online clinical trials?

A remote clinical trial is a research study where some or all of your participation happens away from a study site. Researchers call this a decentralized clinical trial. In practice it can mean video visits with the study doctor, questionnaires completed on your phone, a study medicine delivered by courier, blood draws at a local lab or by a visiting nurse, and a wearable device that sends data automatically. Fully online studies exist, especially surveys, digital therapy programs, and studies of conditions like depression, insomnia, or migraine where symptoms are tracked by diary. Most drug trials are hybrid: a first visit in person for consent and baseline tests, then follow up largely from home.

The model grew fast during the pandemic and stayed. In September 2024 the FDA issued final guidance on how sponsors may run trials with remote elements, covering telehealth visits, home nursing, local labs, and digital data collection, which gave researchers a clear framework and made remote designs standard rather than experimental. The practical result for participants is a wider door: studies that once required living near an academic center can now enroll people hundreds of miles away.
