What is anxiety, and when is it a disorder?

Anxiety is the body's alarm system: a surge of worry, tension, and physical arousal in response to a threat. Everyone has it. It becomes a disorder when the alarm fires too often, too intensely, or without a real trigger, and when it interferes with work, relationships, or daily life for months. The main types are generalized anxiety disorder, with persistent worry about many things; social anxiety disorder, with intense fear of being judged; panic disorder, with sudden attacks of overwhelming fear and physical symptoms; and specific phobias. Anxiety often travels with depression, insomnia, and obsessive compulsive disorder, which has its own page.

Physically, anxiety can feel like a racing heart, chest tightness, shortness of breath, a knot in the stomach, trembling, sweating, or dizziness, which is why it is often mistaken for a heart or breathing problem. A panic attack is the extreme version: minutes of intense fear with strong physical symptoms that peak quickly and pass. Anxiety disorders usually begin in childhood, adolescence, or early adulthood, are more common in women, and run in families, though genes are only part of the story. First line treatment is cognitive behavioral therapy, especially the exposure based kind, and medicines such as SSRIs and SNRIs. They help most people, but a large minority improve only partly, and research is increasingly about matching people to the treatment most likely to work for them rather than trying options one after another.
